Finishing a treatment program is a big accomplishment. It’s also when many people feel least prepared. The structure of rehab goes away overnight, and the pressures of home, work, and old relationships come right back. NMG Recovery Consulting helps clients and families plan for life after rehab and provides the support that makes the plan stick.

Why Aftercare Matters

Treatment gives people tools. Aftercare is where they learn to use them in real life. The first 90 days after discharge are often the hardest, and people who leave with a clear plan and real support behind it tend to do better than those who go home and hope for the best.

Building an Aftercare Plan

We work with the client, the family, and the treatment team, ideally before discharge. Together we map out where the client will live, what their days will look like, which appointments and meetings they’ll attend, who they’ll check in with, and what happens if cravings get strong or they slip.

That plan might include outpatient treatment or therapy, a move to sober living, a return to work or school, and regular accountability. We help coordinate each piece so the client isn’t trying to arrange everything on their own.

Support After Discharge

Aftercare looks different for every client. Some need a sober companion for the first days or weeks at home. Others do well with scheduled recovery coaching and accountability. Clients who aren’t ready to go home can get help finding a good fit through our sober living referrals. If a client starts to struggle, we help arrange a higher level of care through our care coordination and placement service.

Working With Treatment Centers

Treatment centers and detox programs refer clients to us for aftercare and discharge support because we pick up where they leave off. We keep the treatment team informed as the client allows and help carry out the recommendations they made. If you’re a clinician or admissions professional looking for a reliable aftercare partner, we’d welcome a conversation.
